Copy the language strings from modifications.english.php to modifications.yourlanguage.php

In contact page member must enter name, subject, message,e-mail.

I think is better the member enter only  subject and message.

must be default name (forum name) and e-mail address (forum e-mail address)

I like this one.

After trying several other approaches to the contact page issue and not being really satisfied with any of them this one makes me smile.
