Regular members can see any image, if they have a link to it, or can see gallery thumbnails for galleries they do not have permission to view, if they have, or can guess a link.

It correctly excludes (maybe basic smf does this too?) guests.

I was expecting that while viewing a protected category, log out, and back in as a regular forum member, that I would not get to see the protected content.

I have rechecked, and re-set the permissions, no change in behaviour.

Any advice?

How do you have that category setup and what SMF version are you using?
Are you using category level permissions?

I just did a test on SMF 1.1.x and was not able to view a category or view a picture if I set deny on view on category level as guest. If is is a subcategory you will need to add a deny permission for those as well.

Try these files Copy to your sources folder.

Issue was if you had postgroups were considered there own group so a regular membergroup could still be part of a postgroup that was disallowed. Now checks for regular member permission first if denyed.
