Thank you for this.  2 quick questions:  Would it be possible to install this to a custom theme?  And would it be possible to make it for posts instead of topics?

Yes should be very easy to install for custom themes.

For posts would require a minor change to change it to posts instead of topics.

I installed this, but it's count only my "+1'd". Other user of my forum clicks on +1 in topic, and I click too, and it's shows us that only "1 person 1'd this".

It's not counting/showing others click, any suggestions?

Upload package to http://resourcez.donationspro.com/PackageParser/ to get manually install instructions
