Copy the language strings from modifications.english.php to modifications.yourlanguage.php

In contact page member must enter name, subject, message,e-mail.

I think is better the member enter only  subject and message.

must be default name (forum name) and e-mail address (forum e-mail address)

I like this one.

After trying several other approaches to the contact page issue and not being really satisfied with any of them this one makes me smile.   Smiley
Simple, clean and easy to use.
