Download Mod: How to create custome TP search block?

Were can I find the code for this search page?

What I would like to do is create a simple TP download search block.

Something like the ones we have on our site now to search for appliance parts and diagrams.
They can be viewed here on the top left side, http://ApplianceJunk.com/forums

Any help with the code would be greatly appreciated as I'm not a programmer. :)

I would like the TP download search block to be able to , Search Download Title, Search Download Description, but not show them as check box's in the TP block.

I also would not need any of the Advanced Search Options for the TP block.
