Profile Comments: Auto-Double Posting

Hello! I did the install of PROFILE COMMENTS and now I have two profile boxes instead of one where you can add a comment. When you post, two comments are added and deleting one ends up deleting both. Here's what it looks like before:

What coding changes would I need to make to eliminate one of those comment boxes on my BEFORE picture so I won't have to deal with the AFTER picture lol

Looks like the mod was installed twice I would go though the code and remove the duplicate code that was added.
